How is the test performed?

Spirometry is a simple, painless test that usually takes only a few minutes to complete.
It can typically be performed from the age of 4 years, provided that the child is able to cooperate.

During the test:

• a small nose clip is placed on the nose so that the child breathes only through the mouth
• the child is asked to blow forcefully into a special mouthpiece
• simple instructions are given by the examiner.

The manoeuvre is repeated at least twice to ensure reliable results.

In younger children, the test is often accompanied by on-screen animations, where blowing is turned into a game (for example, popping balloons with arrows), making the procedure more enjoyable and engaging.

If necessary, spirometry may be repeated after the administration of an inhaled bronchodilator, in order to assess the lungs’ response to treatment.

When is spirometry needed?

Spirometry is used diagnostically in children with:

• frequent or chronic cough
• episodes of wheezing
• shortness of breath
• reduced exercise tolerance
• recurrent respiratory infections

In addition, it is used for the monitoring of:

• children receiving treatment with inhaled corticosteroids
• children with known lung diseases such as asthma or other chronic respiratory conditions

The measurement of fractional exhaled nitric oxide (FeNO) is a modern, non-invasive method used to detect allergic airway inflammation, as seen in allergic asthma.

The examination is performed in our practice using the Vivatmo Pro, a reliable and child-friendly FeNO measurement system that enables fast and accurate assessment of airway inflammation.

During the procedure, the child takes a deep breath in and then exhales slowly and steadily into the device’s special mouthpiece for a few seconds. Visual guidance on the screen helps the child maintain a constant airflow, ensuring proper and reliable test performance.

The test is generally feasible from the age of 4–5 years, depending on the child’s ability to cooperate. The result is available immediately after the measurement.

FeNO testing is valuable not only for the diagnosis of allergic asthma, but also for monitoring airway inflammation, assessing treatment effectiveness, and adjusting the dosage of inhaled medications when necessary.

For this reason, the test is regularly repeated in children with diagnosed asthma as part of their ongoing follow-up care.

Measurement of Peak Cough Flow (PCF) is particularly important in patients with Neuromuscular diseases. It is used to monitor disease progression and to detect early deterioration of respiratory function. In addition, it helps assess cough effectiveness before and after respiratory infections, supports decision-making regarding the need for mechanical cough assistance (cough assist), and contributes to the evaluation of readiness for weaning from non-invasive ventilation. The test is not performed routinely in healthy children.

Body plethysmography is a specialized pulmonary function test that allows precise measurement of lung volumes and airway resistance. The test is performed in a closed cabin, where the patient breathes through a mouthpiece. Based on pressure changes within the cabin and at the mouth, important parameters such as total lung capacity (TLC), residual volume (RV), functional residual capacity (FRC), and airway resistance (Raw) are calculated.

The examination is painless, involves no radiation exposure, and usually takes about 10–20 minutes.

Body plethysmography is performed when medically indicated, in cooperation with a private clinic equipped with the appropriate technology, and is carried out by prior appointment.

Allergy testing (skin prick testing) is a simple, quick, and safe method used to investigate allergic sensitivities.

During the procedure, small drops of allergen extracts are placed on the skin (usually on the forearm or back). The skin is then gently pricked with a special lancet without causing bleeding.

The test is not painful and may cause only mild itching in case of a positive reaction.

Results are evaluated after approximately 15 – 20  minutes.
The total appointment usually lasts about 30 – 45   minutes.

A provocation test is performed when more detailed evaluation of a suspected allergic reaction (e.g., to foods or medications) is required.

During the test, the child receives gradually increasing, controlled doses of the suspected substance under strict medical supervision.

The procedure is carried out in a safe medical setting, by appointment, in collaboration with a private hospital, with continuous monitoring for any possible symptoms.

The duration typically ranges from 1 to 3 hours, depending on the protocol and type of test.

After completion, the child remains under observation for a short period before discharge.

Telemedicine is particularly suitable for:

• Follow-up visits of established patients
• Discussion of test results
• Monitoring chronic conditions (e.g., asthma)
• Consultation for mild symptoms
• Review or adjustment of treatment plans

However, it does not replace an in-person clinical examination when a physical assessment or urgent medical evaluation is required.

Telemedicine appointments are scheduled in advance and conducted via a certified and secure communication platform.

Bronchoscopy is medically indicated in cases of:

• Suspected foreign body aspiration
• Persistent or unexplained chronic cough
• Recurrent pneumonia in the same lung lobe
• Atelectasis
• Congenital airway abnormalities
• Hemoptysis
• Bronchoalveolar lavage (BAL) for microbiological evaluation

The procedure serves both diagnostic and, in certain cases, therapeutic purposes.Bronchoscopy is performed when medically indicated, by prior appointment, and in cooperation with a private hospital equipped with appropriate technology and anesthesiology support.
